When Corey received notice of an upcoming medical procedure at the VA Clinic, he faced a problem that went far beyond logistics. The procedure would require sedation, meaning he wouldn’t be able to drive himself to or from the appointment. Normally, his wife would have been by his side, but after her recent passing, Corey found himself alone and unsure where to turn for help. 


The VA Clinic referred Corey to Permian Warrior Partnership (PWP) for assistance. That’s when Azzie, PWP’s Case Coordinator, stepped in. Determined to find a solution, she reached out to multiple home health and transportation services in the area. After several calls, Azzie connected with Shelby, a representative from Home Instead home health agency, who immediately expressed a willingness to help.

Together, they worked through the necessary intake process and paperwork to ensure Corey would have safe, reliable transportation to and from his appointments. Just as importantly, the plan made certain that Corey would have someone to see him home and ensure he was comfortable after the procedure.


For Corey, the assistance meant more than just getting to a medical appointment; it meant knowing that someone cared enough to make sure he wasn’t forgotten in a time of need. Thanks to the collaboration between the VA Clinic, Permian Warrior Partnership, and Home Instead, Corey received not only the treatment he needed but also the reassurance that his community was there for him.
